NNCC 2024 QUALITY IMPROVEMENT AWARD WINNER!
|
Elyssa Noce Ely, DNP APRN AGPCNP-C CDCES CNN-NP CGCP of Yale-New Haven Hospital received the 2024 Quality Improvement Award for her project titled: Implementation of an Evidence-Based Educational Toolkit to Enhance Self-Management in Individuals with Chronic Kidney Disease.
Now we will dive deeper into Elyssa’s project and how it has impacted the nephrology community.
There is a lack of consistent self-management support strategies in those with CKD within specialty nephrology care. As such, this project intended to promote a self-

management support strategy in this population to augment medical management.
The purpose of this project was to implement an evidence-based educational toolkit in an ambulatory nephrology practice utilizing a self-management approach to enhance patient knowledge and engagement in the management of their CKD. Chronic kidney disease (CKD) is a complex chronic condition requiring patient engagement in management. Clinical practice guidelines for CKD encourage the use of self-management for improved patient outcomes though there is inconsistent use of self-management support in routine nephrology practice.
An educational toolkit entitled CKD Coach was created with content regarding self-management strategies in the form of short modules for patients with early-stage CKD. Modules were sent to participants over nine weeks. Demographic data and health literacy assessment were collected before education. Assessment of self-efficacy, perception of illness, and knowledge of CKD self-management were collected pre-and post-intervention. A qualitative review of patient experience with the modules was collected post-education.
54 patients completed data collection pre-education, and 46 patients completed data collection post-education. There was a significant increase in self-efficacy, cognitive perception of illness, emotional perception of illness, and understanding of illness following education. There was a significant increase in knowledge in those with and without diabetes. Perceived knowledge increased significantly following education.
CKD Coach was effective in increasing self-efficacy, illness perception, and knowledge in the project population. Implementation of educational programs for patients with CKD could improve self-efficacy, perception, and knowledge and should be used to inspire behavior change in this population to incorporate self-management strategies in their care.

NNCCs Water for Dialysis Program: What's in your Water?

NNCC’s Water for Dialysis Program, What’s in your Water? is a learning activity provided by NNCC to help participants obtain knowledge and competency concerning water treatment for dialysis. Upon completion of this program, participants will be able to:
• Identify the components of the dialysis water system including their purposes, monitoring requirements, and patient safety considerations.

• Explain how water-related clinical problems occur, are identified, and can be prevented.
• Discuss the elements of the CMS water survey that ensure compliance with safety.

Free Practice Exam! Approved CCHT-A Test-Takers!

FREE Online CNN-NP Practice Exam
|
NNCC Board of Commissioners and the Advanced Practice Examination Board have examined some of the roadblocks for nephrology nurse practitioners taking the CNN-NP certification examination.
|
|
|
|
A lack of resources and preparation materials seem to be a large factor. NNCC will now offer a free online CNN-NP practice examination to nephrology nurse practitioners who have been approved to take the CNN-NP examination. Upon approval for testing, nephrology nurse practitioners will receive a link to take the online practice exam. This, in addition to the free CNN-NP Certification Preparation Guide, will have you well on your way to preparing for the exam!
